INDIVIDUAL INVESTORS - LI (RESIDENT SRI LANKANS)
Required Documents
- A clear photo copy of the National Identity Card (NIC).
- If the NIC is not available a copy of a valid Passport as at the date of the Account being opened at the CDS.
- If the account is a joint account, a clear cop of the NIC/ Passport of both clients should be provided.
- Copy of most recent utility bill or bank/credit card statement received to the residential address given in the application received in the past three months (if the permanent address differs from the Address given on the NIC).
- Bank Book / Bank Statement to prove the bank account mentioned.

INDIVIDUAL INVESTORS - FI (NON RESIDENT SRI LANKANS)
Required Documents
- A copy of valid Sri Lankan Passport
- Copy of Utility bill / Credit Card Statement / Bank Statement issued within the last three months

INDIVIDUAL INVESTORS - FR (NON SRI LANKANS)
Required Documents
- A copy of valid Passport.
- Copy of Utility Bill / Credit Card Statement / Bank Statement issued within the last three months
- Proof of Permanent/Residential Address
- SIA (Securities Investment Account) details with documentary proof.
Required Forms
INSTITUTIONAL INSINVESTORS - LC (LOCAL COMPANIES)
Required Documents
- A copy of the Certificate of Incorporation of the company.
- If the company has changed its name, a copy of the Certificate of Name Change issued by the Registrar of Companies is also required.
- A copy of the Articles of Association of the company.
- An extract of the resolution passed by the Board of Directors to open the CDS account.
- Alternatively the company may provide a certification in the relevant section in the account opening form (CDS 2).
- An extract of the resolution passed by the Board of Directors stating the full names and specimen signature/s of the authorized signatory/ies to give instructions to Acuity.
- A letter issued within 12 months by the Company Secretary indicating the names, addresses, NIC or Passport numbers and occupations of all existing directors & top shareholders (names and addresses) up to a maximum of ten shareholders.
- Bank Book / Bank Statement to prove the bank account mentioned.

INSTITUTIONAL INSINVESTORS - FC (FOREIGN COMPANIES)
Required Documents
- A copy of the Certificate of Incorporation of the company or equivalent document, issued by the Registrar of Companies/applicable authority where the company is incorporated.
- If the company has changed its name a copy of the Certificate of Name Change issued by the Registrar of Companies/applicable authority where the company is incorporated is also required.
- A copy of the Articles of Association of the company or equivalent document.
- A copy of the Certificate of Good Standing issued within a period of one year by the Registrar of Companies/applicable authority where the company is incorporated.
- A certified extract of the resolution passed by the Board of Directors to open the CDS account.
- An extract of the resolution passed by the Board of Directors stating the full names and specimen signature/s of the authorized signatory/ies to give instructions to Acuity.
- In the alternative the company may provide a certification in the relevant section in the account opening form itself (CDS 2).
- Certificate to Commence Business issued by the Registrar of Companies/applicable authority where the company is incorporated. (where applicable)
- A letter issued by the Company Secretary giving the names, address and Passport Number (passport copy) of directors and the name of the listed Stock Exchange and web site reference to the top 10 shareholders.
- SIA (Securities Investment Account) details with documentary proof
